Network Services Telephone - VoIP Telephone Service

This webform is designed to help you with your VoIP device service request.  Please provide your customer information, then tell us how we can help you by making a selection from the provided choices of service types.  Additional questions will show up as you make selections.

New Mitel VoIP Telephone instruments are to be purchased by the Department from the Bookstore.  The standard for new service is Mitel 6940.

When you receive your instrument from the Bookstore, provide to networkservices@louisiana.edu the
Mitel telephone instrument's model number and MAC address so that the programming can be completed.
